Standard Registered Nurse visit. Includes assessment, recommendations, and patient education.
RN Visit +

RN visit with testing and/or medication given.
Follow-Up RN Visit

Scheduled if follow-up assessment is required.
Physical Exam
$75
Pre-participation physical exam with Nurse Practitioner. Only offered during clinics.
Med Eval - Initial
$75
Behavioral health medication evaluation with Nurse Practitioner. Must have a counselor referral; appointments limited.
Med Eval - Follow-up
$25
Behavioral health med follow-up with NP. Required when medication is prescribed.
