Medical Record Requests

To request medical records:

Please note: Student medical records are kept for 7 years post-graduation or separation from the College.

If you were a student between Fall 2018 and Spring 2025, request your medical record from MFM Health.

Students who uploaded Medical Requirement Forms to their admissions portal within the past 7 years may request a copy of their forms.

Please note: Requests made outside of the academic year may require longer processing times. Medical Requirement Forms will not be emailed, but can be sent by mail or faxed to a current medical provider or academic institution.

The Health Center is located on the first floor of Lane Student Center, to the left of the mailroom.

Nursing Hours by appointment only:
Monday–Thursday 9 a.m.–2 p.m. 
Friday 9 a.m.–noon 
The on-campus Health Center is closed during school breaks and summer.
