3. Review the Course Packet
The course packet is an important resource that includes information about the Expedition, how to prepare, and a checklist with due dates for items that need to be completed.
4. Complete Medical Forms in your Admissions Student Portal
La Vida has access to your Medical Forms in the Student Portal. Please complete them by two weeks before your trip.
- Medical Requirements Checklist: the Medical Questionnaire and Medical Requirements Form are both located in your Student Portal
- Medications Authorization Form: If you will be under the age of 18 during the Expedition and plan to take any medication, please fill out this attached form
5. Finish Precourse Paperwork
Please fill out all paperwork listed below by one month prior to your Expedition.
- La Vida Expedition Release Forms: Include proof of insurance
- Essential Eligibility Criteria Response Form: Full description and explanation here
- Food Form: Allergy and Dietary Need Details: This form will help staff prepare food for the Expedition
- Arrival and Departure Form: Please let us know your travel plans
6. Read the Packing List and gather gear
Review the packing list to see a thorough description of what type of gear is needed. You will also be able to borrow some equipment from the La Vida Base Camp before the start of your Expedition.
