Optional Immunization Forms

Students requesting a religious or medical immunization exemption must complete both the Request for Immunization Exemption Form and the Immunization Exemption Acknowledgment Form. Those seeking medical immunization exemption must also submit documentation signed by a medical provider. 

Incoming students, please upload all forms and documentation to the Admissions portal, along with the completed Medical Requirements Form.

Returning students, please submit documentation directly to the Health Center by mail, fax, or in person. Do not email medical documents as email is not secure, and emailed documents will not be accepted.

The Health Center is located on the first floor of Lane Student Center, to the left of the mailroom.

Nursing Hours by appointment only:
Monday–Thursday 9 a.m.–2 p.m. 
Friday 9 a.m.–noon 
The on-campus Health Center is closed during school breaks and summer.
