Compass Mentor Form

The Mentor Form is to be completed by someone who has committed to mentoring the student for three years following the program. Every student applying for the program needs to have a mentor.

Wondering who could be a mentor? A mentor should be a pastor, teacher, coach, Bible study leader or another adult who desires to mentor the student in the program. For more on selecting a mentor, visit our FAQ page.
