Gourmet Boxed Lunch Order Form 2014

NEW GOURMET BOXED LUNCH PROGRAM FOR WORKSHOP PARTICIPANTS
We are pleased to offer summer workshop participants the opportunity to order Pre-Packaged Gourmet Boxed Lunches at a cost of $9.50 per lunch. Lunches are delivered daily directly to a central workshop classroom building location. Complete menu is available on the order form below. All lunches include the following for $9.50:
chips
whole fruit
cookie or brownie
condiments
fork, knife, napkin
assorted chilled soda and bottled water

Orders must be submitted online one week in advance. Example: if you would like to order a boxed lunch for Monday, July 14, you must submit your order online by noon on Monday, July 7.

We currently do not have a secure set up for taking credit card payments online. An invoice will be generated at the end of workshop week for all meals ordered, and it will be mailed to your home address. Failure to remit payment will result in holding the release of graduate credit grade reports, PDP and other workshop participation documentation.

ON CAMPUS RESIDENTS AND MEAL PLAN PARTICIPANTS
For those participants who are staying in on campus housing, the boxed lunches are included as part of your optional meal plan selection. If you have chosen to add breakfast and/or dinner as part of your meal plan, meal vouchers will be provided for you to use in the main dining hall located on the main level of Lane Student Center .

9. Billing Agreement
By entering my initials below, I have read and understand that upon completion of the Graduate Music Workshop Week Program, an invoice for meals ordered will be generated and mailed to my home address. I agree to submit payment by mail within thirty days of receipt of the invoice. I understand that failure to do so will result in the withholding of grade reports, PDP documentation, and program attendance verification.
